It was almost certainly caused by something that is incorrect in the second photo Ivor.

Mmmh, the locking tabs haven’t been turned over, but the nuts are still there...This could be a two pipe problem or I’m being a complete twit...

The thrust bearings have been installed the incorrect way around.
