Here is one I made with the feature described in the earlier answers. It is fitted to the gearbox with longer than standard bolts so that it does not strip the threads, and the collar around the original top column of the gearbox is a close fit. There is no bolting down to the transmission tunnel; it is rigidly fixed only to the gearbox. Halfway along the tube there is a solid nylon bock fixed inside to the bottom so that pushing the gear lever down lifts the front end to select reverse. I think the gear lever pivot came from the top of a Ford Escort gearbox.
