Unfortunately the shoulder is too small to enable recovery by grinding. The  wear encountered on some cars was remarkable. I have seen them worn into the steel of the axle. Despite the traditions of the time many owners never greased cars, or troubled to unblock drillings (The old greases like Castrol CL dried; formed a useful crust but more prone to blockage than modern).
Uses of are limited although I have used for levering

The choice of steel grade for this application is definitely EN24 or 4340. The steel is inexpensive and easy to machine. The pins should be ground after heat treatment to give long service life. Harden and temper back to 48 to 50HRc. Similar comment for king pins

Presumably anyone making would use separate endplates or drill the old and use double ended pins. i understand for kingpin safety  some use hard chrome plated HT rod as for hydrauics. That eliminates the heat treatment ritual
